Annual Block Fee Program 2026
Uninsured Services and all Third Party requested forms are not covered by the Provincial Health Insurance Plan (OHIP) and will be charged to the patient.
These rates are outlined by the Ontario Medical Association and are charged accordingly.
We offer a “block fee” payment that will cover or discount uninsured services on a yearly basis. This “block fee” is paid in lieu of the individual paying for each uninsured service.
The Ontario Health Insurance Plan (OHIP) covers the majority of physician services, but there are certain services that are not included. Your HMC physician offers these valuable services, but the patient or the third party making the request is responsible for payment.
The Annual Block Fee program is a flat, yearly membership available to patients registered with the High Metabolic Clinic and is an OPTIONAL way to cover costs for services not covered by OHIP. We encourage patients to sign up for the Annual Block Fee program as it will make the process of requesting and receiving the most common uninsured services easier by offering coverage with one simple fee.
What uninsured services are covered by the Annual Block Fee?
Prescription renewals without a visit ($30 per request)
Insurance forms ($80-110 per request) usually required every 6 months
Transfer of medical records ($30)
Requests for copies of medical records, including bloodwork ($30)
Online appointment booking ($40)
Doctor’s letters (for travel or any other purpose) $60
Our Annual Block Fee is $125 CAD.
Enrolling in the Annual Block Fee program gives you unlimited access to the services above for 12 months, however it is entirely voluntary. If you choose not to join, you can still access these services on an a-la-carte basis whenever you need them. Please note that the Annual Block Fee will not be applied retroactively to fees already paid.
